1. The initiator will receive an email notification upon submission of the request
2. Human Resources will research and verify the information submitted.
   • Upon validation, Sections 3 and 4 will be filled out by a Human Resources representative and prepared according to the delivery method.
     • The ticket will be closed and the ticket initiator will be notified the request has been processed and sent out for delivery
   • If the information cannot be validated, the initiator will be notified and the ticket will be closed out.
