Undergraduate Certificates are not official University credentials and are not eligible for a diploma/credential.
Before submitting this form, review your Academic Advising Report with your academic advisor.
Your Advising Report for the certificate must show all requirements complete with in-progress work.
Your certificate will be posted on your transcript at the end of the semester if all certificate requirements have been met.
Your name will not appear in any graduation publications nor are you eligible to participate in commencement.
