Formstack Forms is the web form management tool the University uses on the main University website (www.unr.edu). It allows you to build custom online forms which can be embedded or sent out and filled in by participants. This service is managed by the Office of Marketing and Communications.
Requirements
Users are required to complete Basic Web Accessibility Training before access can be granted.
Access and Support
Get Formstack
To request access, please complete the Formstack Security Request Form (requires NetID login). Requests for access require approval; submissions are routed to the Dean or Vice President of the user’s College or Division.
