The re-evaluation is based on updated transcripts. Please submit these official documents to Admissions and Records at the University of Nevada, Reno prior to submitting this form to ensure it will be processed in a timely manner.
Please note: Transcripts must come directly from the school or institution to us to be considered official. Unofficial transcripts or progress reports cannot be accepted.

This form cannot be used after the start of the term in which you applied. You will be required to submit a new application for a different term if this is the case. 
Once this form is submitted, our staff will review your request and updated work. Allow 10 - 15 business days for an update on your re-evaluation request.
