This form is intended to be used by those that are applying to the University and meet the following criteria:
- Has already graduated high school and
- Attended another post-secondary institution after graduation but did NOT attempt/complete 24 or more credits
Only those that receive the option to request this change in their denial letter should complete this form.
Please note that by completing this form, you are requesting to be considered for admissions based on our Freshman admissions standards and will be required to submit your final high school transcript.
