General Student Inquiry

Incoming Freshman Change of Major Request

This form is for incoming freshman for an upcoming term. This form is NOT for current students in attendance that are wishing to change a previously declared major.

Math/English Enrollment Assistance Request

Students wishing to make changes to their Math/English course enrollment must fill out this request.

NSHE Transcript Request form

tudents who are sending transcripts to NSHE institutions can do so free of charge.

The institutions that qualify for free transcript delivery include the following:
College of Southern Nevada (CSN)
Great Basin College (GBC)
Nevada State College (NSC)
Truckee Meadows Community College (TMCC)
University of Nevada, Las Vegas (UNLV)
Western Nevada College (WNC)

Request for Re-evaluation

Complete this form if you are requesting an application re-evaluation for a denied undergraduate degree program application.

Request to Change to Non-Degree Seeking After Dismissal

Students who are on Notice of Academic Dismissal and earn a UNR term GPA of below 2.0 will be dismissed from the University at the end of the term.

An undergraduate student who has been academically dismissed may not enroll as a degree-seeking student for one academic year.

Students can utilize this form to change their status to non-degree seeking to continue to take classes at the University.

Request to Remove: Major, Minor or Certificate

Students who wish to remove a major, minor or certificate program should use this form.

Returning Student Application

If you are a student who has previously attended the University of Nevada, Reno but have had a lapse in enrollment, you must complete the Returning Student Re-Activation form to be considered for admission.

Transcript Request Support

Assistance for current or former students on how to order an official or unofficial transcript, checking the status of an order placed, or why an order is not able to be processed.
