This form is for incoming freshman for an upcoming term. This form is NOT for current students in attendance that are wishing to change a previously declared major.

Please note that by changing your major your academic advisor may be:

Changing your Fall 2026 academic schedule to align with major requirements
Changing your Pre-Advising College Course

These adjustments will ensure that you are in the correct courses for your major and are prepared to meet with the correct advising team.
