Use this service to request access to SQL Server.

• New - If you are requesting access for the first time.
• Change Current - If you are requesting a change to your existing access for the same department/position.
• Name Change - If you are requesting a name change to your existing access.
• Department Transfer - If you are requesting a change to your existing access due to a transfer to another department​​​​​​.
• Elevate Permissions Temporarily - If you are requesting to elevate permissions temporarily to your existing access.
