  1. Click Request Service button to the right to be redirected to the form
  2. Complete the form in TeamDynamix
  3. The person submitting the request receives a notification that a ticket has been created.
  4. Human Resources will perform analysis pursuant to standard procedures and reach out to the requester directly upon approval or if additional information is needed.
  5. To inquire about your ticket's status, open the ticket and make a comment on it. You must notify HR Services when placing your comment or Human Resources will not be notified.

Note: The ticket will automatically be put into a Closed status. This means that Human Resources has been made aware of your request and is working on it; it does not mean your ticket has been cancelled or closed without action.
