- Click Request Service button to the right to be redirected to the form
- Complete the form in TeamDynamix
- The person submitting the request receives a notification that a ticket has been created.
- Human Resources will perform analysis pursuant to standard procedures and reach out to the requester directly upon approval or if additional information is needed.
- To inquire about your ticket's status, open the ticket and make a comment on it. You must notify HR Services when placing your comment or Human Resources will not be notified.

Note: The ticket will automatically be put into a Closed status. This means that Human Resources has been made aware of your request and is working on it; it does not mean your ticket has been cancelled or closed without action.
