Complete this form if you are requesting an application re-evaluation for a denied undergraduate degree program application. The re-evaluation is based on updated transcripts and/or new test scores. Please submit these official documents to Admissions and Records at the University of Nevada, Reno prior to submitting this form to ensure it will be processed in a timely manner. Unofficial copies of transcripts or test scores are not acceptable for a re-evaluation.

This form is intended to be used by transfer applicants that have not attempted/completed 24 or more transferrable credits and are wishing to have their application type switched to "freshman with transfer credits"

Used to process requests to add or remove ES staff from TeamDynamix and ES ticketing groups.
This appeal is only for students who have previously paid the graduation application fee for the same degree in a previous term.
Students who are on Notice of Academic Dismissal and earn a UNR term GPA of below 2.0 will be dismissed from the University at the end of the term.
An undergraduate student who has been academically dismissed may not enroll as a degree-seeking student for one academic year.
Students can utilize this form to change their status to non-degree seeking to continue to take classes at the University.
