This form is for incoming freshman for an upcoming term. This form is NOT for current students in attendance that are wishing to change a previously declared major.
INCOMING TRANSFER USE ONLY
Complete this form if you are requesting reconsideration to qualify for the Western Undergraduate Exchange (WUE) or the Pack Exchange Program (PEP).
Students who are on Notice of Academic Dismissal and earn a UNR term GPA of below 2.0 will be dismissed from the University at the end of the term.
An undergraduate student who has been academically dismissed may not enroll as a degree-seeking student for one academic year.
Students can utilize this form to change their status to non-degree seeking to continue to take classes at the University.
This form is intended to be used by transfer applicants that have not attempted 24 transferrable credits and are wishing to have their application type switched to "freshman with transfer credits"
Questions from current and former students regarding balance owed, account past due hold, fees assessed, or needing a copy of 1098T form.
Assistance for instructors needing to complete FERPA training or academic departments who need student workers/TAs be added into the FERPA training course.
Students who wish to remove a major, minor or certificate program should use this form.

If you are a student who has previously attended the University of Nevada, Reno but have had a lapse in enrollment, you must complete the Returning Student Re-Activation form to be considered for admission.
Students wishing to make changes to their Math/English course enrollment must fill out this request.
