Complete this form if you are requesting an application re-evaluation for a denied undergraduate degree program application. The re-evaluation is based on updated transcripts and/or new test scores. Please submit these official documents to Admissions and Records at the University of Nevada, Reno prior to submitting this form to ensure it will be processed in a timely manner. Unofficial copies of transcripts or test scores are not acceptable for a re-evaluation.
Students who are on Notice of Academic Dismissal and earn a UNR term GPA of below 2.0 will be dismissed from the University at the end of the term.
An undergraduate student who has been academically dismissed may not enroll as a degree-seeking student for one academic year.
Students can utilize this form to change their status to non-degree seeking to continue to take classes at the University.
