Please submit these official documents to Admissions and Records at the University of Nevada, Reno prior to submitting this form to ensure it will be processed in a timely manner. Unofficial copies of transcripts or test scores are not acceptable for a re-evaluation.
This form is for incoming freshman for an upcoming term. This form is NOT for current students in attendance that are wishing to change a previously declared major.

Complete this form if you are requesting reconsideration to qualify for the Western Undergraduate Exchange (WUE) or the Pack Exchange Program (PEP).

Academic department schedulers who need assistance in scheduling a course, assigning an instructor to a course, or troubleshooting grade roster access.
If you are a student who has previously attended the University of Nevada, Reno but have had a lapse in enrollment, you must complete the Returning Student Re-Activation form to be considered for admission.
Students who are on Notice of Academic Dismissal and earn a UNR term GPA of below 2.0 will be dismissed from the University at the end of the term.
An undergraduate student who has been academically dismissed may not enroll as a degree-seeking student for one academic year.
Students can utilize this form to change their status to non-degree seeking to continue to take classes at the University.
Please submit this form if you have met with your current academic advisor and no longer wish to participate in the Silver State Transfer Program.
